De asemenea, acest drept se extinde \u0219i la alte categorii precum asocia\u021bii, comanditari, persoane independente sau cele care au un contract de asigur\u0103ri sociale.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Persoanele asigurate pentru concedii \u0219i indemniza\u021bii de asigur\u0103ri sociale de s\u0103n\u0103tate \u00een sistemul de asigur\u0103ri sociale de s\u0103n\u0103tate, denumite \u00een continuare asigura\u021bi, au dreptul, \u00een condi\u021biile prezentei ordonan\u021be de urgen\u021b\u0103, pe perioada \u00een care au domiciliul sau re\u0219edin\u021ba pe teritoriul Rom\u00e2niei, la concedii medicale \u0219i indemniza\u021bii de asigur\u0103ri sociale de s\u0103n\u0103tate, dac\u0103:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">desf\u0103\u0219oar\u0103 activit\u0103\u021bi pe baz\u0103 de contract individual de munc\u0103\/raport de serviciu\/act de deta\u0219are, precum \u0219i alte venituri asimilate salariilor<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">beneficiaz\u0103 de drepturi b\u0103ne\u0219ti lunare ce se suport\u0103 din bugetul asigur\u0103rilor pentru \u0219omaj<\/span><\/li>\n<\/ul>\n<h2><b>Tipuri de concedii medicale<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Concediile medicale la care dreptul asigura\u0163ii sunt:<br \/>\n<\/span><\/p>\n<p>a) concedii medicale \u015fi indemniza\u0163ii pentru incapacitate temporar\u0103 de munc\u0103, cauzat\u0103 de boli obi\u015fnuite sau de accidente \u00een afara muncii;<br \/>\nb) concedii medicale \u015fi indemniza\u0163ii pentru prevenirea \u00eemboln\u0103virilor \u015fi recuperarea capacit\u0103\u0163ii de munc\u0103, exclusiv pentru situa\u0163iile rezultate ca urmare a unor accidente de munc\u0103 sau boli profesionale;<br \/>\nc) concedii medicale \u015fi indemniza\u0163ii pentru maternitate;<br \/>\nd) concedii medicale \u015fi indemniza\u0163ii pentru \u00eengrijirea copilului bolnav;<br \/>\ne) concedii medicale \u015fi indemniza\u0163ii de risc maternal;<br \/>\nf) concedii medicale pentru ingrijire bolnav oncologic cod 17.<\/p>\n<h2><b>Calculul indemniza\u021biei de concediu medical<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Indemniza\u021bia de concediu medical se calculeaz\u0103 pe baza veniturilor brute din ultimele 6 luni, plafonat\u0103 la 12 salarii minime brute pe \u021bar\u0103. Se determin\u0103 o medie zilnic\u0103 din aceste venituri, care, \u00eenmul\u021bit\u0103 cu num\u0103rul de zile de concediu medical \u0219i procentul specific fiec\u0103rui tip de concediu, stabile\u0219te cuantumul indemniza\u021biei.<\/span><\/p>\n<p><i><span style=\"font-weight: 400;\">Exemplu:&nbsp;concediul medical de 5 zile, cu cod de indemniza\u021bie de 100%, cu urm\u0103toarea baz\u0103 de calcul:<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400;\">Baz\u0103 calcul = 11130 lei,<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Zile baz\u0103 calcul= 126<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Media zilnic\u0103 = 9653 \/ 126 = 88,33 lei<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Cuantumul indemniza\u021biei = 88.33 lei * 5 zile = 441,66 lei<\/span><\/p>\n<p><i><span style=\"font-weight: 400;\">Not\u0103: Aici avem cod cu procent de 100%. --smush-placeholder-aspect-ratio: 1024\/772;\" \/><\/a><\/b><\/h2>\n<h2><b>Procentele de plat\u0103 dup\u0103 codurile de indemniza\u021bie<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Fiecare tip de concediu medical este asociat cu un cod de indemnizatie, fiecare av\u00e2nd un procent specific de plat\u0103 din venitul brut, variind de la 75% la 100%, \u00een func\u021bie de natura afec\u021biunii.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">01 &#8211; Boala obi\u0219nuit\u0103 &#8211; 75%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">02 &#8211; Accident \u00een timpul deplas\u0103rii la\/de la locul de munc\u0103 &#8211; 100%\/80%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">03 &#8211; Accident de munc\u0103 &#8211; 100%\/80%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">04 &#8211; Boala profesional\u0103 &#8211; 100%\/80%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">05 &#8211; Boala infectocontagioas\u0103 din grupa A &#8211; 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">51 \u2013 Boala infec\u021biocontagioas\u0103 pentur care se instituie m\u0103sura izol\u0103rii \u2013 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">06 &#8211; Urgen\u021b\u0103 medico-chirurgical\u0103 &#8211; 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">07 &#8211; Carantina &#8211; 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">08 &#8211; Sarcina \u0219i l\u0103uzie &#8211; 85%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">09 &#8211; \u00cengrijire copil bolnav \u00een v\u00e2rsta de pana la \u0219apte ani sau copil cu handicap pentru afec\u021biuni intercurente, pana la \u00eemplinirea v\u00e2rstei de 18 ani &#8211; 85%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">91- \u00cengrijire copil bolnav cu afec\u021biuni grave \u00een v\u00e2rst\u0103 p\u00e2n\u0103 la 18 ani \u2013 85%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">92 \u2013 Supravegherea \u0219i \u00eengrijirea copilului \u00een v\u00e2rst\u0103 de p\u00e2n\u0103 la 18 ani pentru care s-a dispus m\u0103sura carantinei sau a izol\u0103rii \u2013 100%&nbsp;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">10 &#8211; Reducerea cu 1\/4 a duratei normale de lucru &#8211; F\u0103r\u0103 a dep\u0103\u0219i 25% din baza de calcul<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">11 &#8211; Trecerea temporar\u0103 \u00een alt\u0103 munc\u0103 &#8211; F\u0103r\u0103 a dep\u0103\u0219i 25% din baza de calcul<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">12 &#8211; Tuberculoz\u0103 &#8211; 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">13 &#8211; Boala cardiovascular\u0103 &#8211; 75%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">14 &#8211; Neoplazii, SIDA &#8211; 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">15 &#8211; Risc maternal &#8211; 75%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">16 Unele tipuri de arsuri, inclusiv pentru perioada de recuperare \u2013 100%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">17 \u00cengrijire pacient cu afec\u021biuni oncologice \u2013 85%<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u00cen plus este important s\u0103 punct\u0103m c\u0103:<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pl\u0103titorul, respectiv angajatorul, se oblig\u0103 s\u0103 respecte confiden\u021bialitatea diagnosticului.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Persoana asigurat\u0103 sau reprezentantul legal sau mandatarul desemnat prin procur\u0103 se oblig\u0103 s\u0103 anun\u021be pl\u0103titorul despre acordarea concediului medical \u015fi s\u0103 depun\u0103 certificatul de concediu medical p\u00e2n\u0103 cel mai t\u00e2rziu la data de 5 a lunii urm\u0103toare celei pentru care a fost acordat concediul medical.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Calculul indemniza\u021biei de asigur\u0103ri sociale de s\u0103n\u0103tate se efectueaz\u0103 conform normelor legale \u00een vigoare.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pentru codurile de indemniza\u021bie 02, 03, 04, 10, confirmate prin Avizul casei teritoriale de pensii sau Avizul direc\u021biei de s\u0103n\u0103tate public\u0103, precum \u015fi pentru codul de indemniza\u021bie 11 certificatele se depun la casele teritoriale de pensii.<\/span><\/p>\n<h2><b>Responsabilitatea pl\u0103\u021bii indemniza\u021biei<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Primele 5 zile de concediu medical sunt suportate de angajator, iar de la a \u0219asea zi, plata se face din Fondul Na\u021bional Unic de Asigur\u0103ri Sociale de S\u0103n\u0103tate. Exist\u0103 categorii de concedii medicale care sunt pl\u0103tite integral din fonduri FNUASS, respectiv:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">indemniza\u021bia de maternitate<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">indemniza\u021bia pentru \u00eengrijirea copilului bolnav<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">indemniza\u021bia de risc maternal<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">indemniza\u021bia pentru reducerea timpului de munc\u0103<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">indemniza\u021bia pentru carantin\u0103<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">indemniza\u021bia pentru \u00eengrijirea pacientului cu afec\u021biuni oncologice \u00een v\u00e2rst\u0103 de peste 18 ani.<\/span><\/li>\n<\/ul>\n<h2><b>Eliberarea certificatelor de concediu medical<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Certificatele de concediu medical pot fi eliberate de:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medicul primar<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Orice medic cu autoriza\u021bie&nbsp;de liber\u0103 practic\u0103 valabil\u0103, care are \u00een \u00eengrijire \u0219i sub monitorizare bolnavul \u0219i care are \u00eencheiat\u0103 o conven\u021bie cu o cas\u0103 de asigurare.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medicul de familie&nbsp;\u2013 acesta poate acorda certificate de concediu medical de 1 &#8211; 14 zile, \u00een una sau mai multe etape.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medicul primar sau specialist din ambulatoriul de specialitate\/spital&nbsp;poate prelungi concediul medical \u00een etape succesive. El va putea acorda prelungiri de maximum 31 zile p\u00e2n\u0103 la totalul de 90 zile calendaristice \u00een decurs de un an (anul se socote\u0219te de la prima zi de \u00eemboln\u0103vire).<\/span><\/li>\n<\/ul>\n<h2>Concediul \u015fi indemniza\u0163ia pentru incapacitate temporar\u0103 de munc\u0103 cauzat\u0103 de boli obi\u015fnuite sau de acccident \u00een afara muncii- cod indemniza\u0163ie 01, 05, 06, 12, 13, 14.<\/h2>\n<p>Durata de acordare a concediului \u015fi a indemniza\u0163iei pentru incapacitate temporar\u0103 de munc\u0103, pentru fiecare tip de afec\u0163iune, este de cel mult 183 de zile \u00een interval de un an, socotit din prima zi de \u00eemboln\u0103vire. \u00cencep\u00e2nd cu a 91-a zi concediul se poate prelungi, de c\u0103tre medicul specialist, p\u00e2n\u0103 la 183 de zile, cu aprobarea medicului expert al asigur\u0103rilor sociale.<br \/>\nConcediul \u0219i indemniza\u0163ia pentru incapacitate temporar\u0103 de munc\u0103, cauzat\u0103 de boli obi\u015fnuite sau de acccident \u00een afara muncii- cod indemniza\u0163ie 01, 05, 06, 12, 13, 14.<br \/>\nCuantumul brut lunar al indemniza\u0163iei pentru incapacitate temporar\u0103 de munc\u0103, prin boal\u0103 obi\u015fnuit\u0103, rezult\u0103 \u00een urma produsului dintre media zilnic\u0103 a bazei de calcul \u015fi procentul de 75% duplicat cu num\u0103rul de zile lucr\u0103toare de concediu medical.<br \/>\nCuantumul brut lunar al indemniza\u0163iei pentru incapacitate temporar\u0103 de munc\u0103, determinat\u0103 de tuberculoz\u0103, SIDA, neoplazii, precum \u015fi de o boal\u0103 infectocontagioas\u0103 din grupa A \u015fi de urgen\u0163e medico-chirurgicale stabilite \u00een condi\u0163iile prev\u0103zute la art. 9, rezult\u0103 \u00een urma produsului dintre media zilnic\u0103 a bazei de calcul \u015fi num\u0103rul de zile lucr\u0103toare de concediu medical.&nbsp;<\/p>\n<h2>Concediile \u015fi indemniza\u0163iile pentru prevenirea \u00eemboln\u0103virilor \u015fi recuperarea capacit\u0103\u0163ii de munc\u0103- cod indemniza\u0163ie 10 \u015fi 07<\/h2>\n<p>\u00cen scopul prevenirii \u00eemboln\u0103virilor \u015fi recuper\u0103rii capacit\u0103\u0163ii de munc\u0103, asigura\u0163ii pot beneficia de:<br \/>\na) indemniza\u0163ie pentru reducerea timpului de munc\u0103;<br \/>\nb) concediu \u015fi indemniza\u0163ie pentru carantin\u0103;<br \/>\nc) tratament balnear, \u00een conformitate cu programul individual de recuperare.<\/p>\n<h2>Concedii medicale de maternitate, pentru \u00eengrijirea copilului bolnav \u015fi de risc maternal<\/h2>\n<p>Concediul \u015fi indemniza\u0163ia de maternitate- cod indemniza\u0163ie 08.<\/p>\n<p>Asiguratele au dreptul la concedii pentru sarcin\u0103 \u015fi l\u0103uzie, pe o perioad\u0103 de 126 de zile calendaristice, perioad\u0103 \u00een care beneficiaz\u0103 de indemniza\u0163ie de maternitate.<br \/>\nDe acelea\u015fi drepturi beneficiaz\u0103 \u015fi femeile care nu se mai afl\u0103, din motive neimputabile lor, \u00een una dintre situa\u0163iile prev\u0103zute la art. anterior, dac\u0103 nasc \u00een termen de 9 luni de la data pierderii calit\u0103\u0163ii de asigurat. Faptul c\u0103 pierderea calit\u0103\u0163ii de asigurat nu s-a produs din motive imputabile persoanei \u00een cauz\u0103 se dovede\u015fte cu acte oficiale eliberate de c\u0103tre angajatori sau asimila\u0163ii acestora.<\/p>\n<h2>Concediul \u015fi indemniza\u0163ia pentru \u00eengrijirea copilului bolnav- cod indemniza\u0163ie 09<\/h2>\n<p>Asigura\u0163ii au dreptul la concediu \u015fi indemniza\u0163ie pentru \u00eengrijirea copilului bolnav \u00een v\u00e2rst\u0103 de p\u00e2n\u0103 la 7 ani, iar \u00een cazul copilului cu handicap, pentru afec\u0163iunile intercurente, p\u00e2n\u0103 la \u00eemplinirea v\u00e2rstei de 18 ani.<br \/>\nDurata de acordare a indemniza\u0163iei prev\u0103zute la art.27, alineatul 1 din OUG 158\/2005 este de maximum 45 de zile calendaristice pe an, pentru un copil, cu excep\u0163ia situa\u0163iilor \u00een care copilul este diagnosticat cu boli infectocontagioase, neoplazii, este imobilizat \u00een aparat gipsat, este supus unor interven\u0163ii chirurgicale; durata concediului medical \u00een aceste cazuri va fi stabilit\u0103 de medicul curant, iar dup\u0103 dep\u0103\u015firea termenului de 90 de zile, de c\u0103tre medicul specialist, cu aprobarea medicului expert al asigur\u0103rilor sociale.<\/p>\n<h2>Concediul \u015fi indemniza\u0163ia de risc maternal- cod indemniza\u0163ie 15<\/h2>\n<p>Dreptul la concediul de risc maternal se acord\u0103 \u00een condi\u0163iile prev\u0103zute de <a href=\"https:\/\/legislatie.just.ro\/Public\/DetaliiDocumentAfis\/47216\" target=\"_blank\" rel=\"noopener\">Ordonan\u0163a de urgen\u0163\u0103 a Guvernului nr. 96\/2003<\/a> privind protec\u0163ia maternit\u0103\u0163ii la locurile de munc\u0103, aprobat\u0103 cu modific\u0103ri \u015fi complet\u0103ri prin Legea nr. 25\/2004, cu modific\u0103rile ulterioare.<br \/>\nCertificatele de concediu de risc maternal se elibereaz\u0103 de medicul de familie sau medicul de specialitate obstetric\u0103-ginecologie, cu avizul medicului de medicina muncii, pe perioade de maximum 30 de zile calendaristice, pe o durat\u0103 total\u0103 de maximum 120 de zile calendaristice, \u00een \u00eentregime sau frac\u0163ionat, asiguratelor gravide, asiguratelor care au n\u0103scut recent sau care al\u0103pteaz\u0103.<\/p>\n<h2>Concediul medical pentru \u00eengrijirea pacientului cu afec\u021biuni oncologice \u00een v\u00e2rst\u0103 de peste 18 ani &#8211; cod indemniza\u021bie 17<\/h2>\n<div>Av\u00e2nd \u00een vedere prevederile Ordinului&nbsp; <a href=\"https:\/\/legislatie.just.ro\/Public\/DetaliiDocumentAfis\/254232\" target=\"_blank\" rel=\"noopener\">MS\/CNAS nr. 1.166\/217\/2022<\/a> pentru modificarea \u0219i completarea <a href=\"https:\/\/legislatie.just.ro\/Public\/DetaliiDocument\/196683\" target=\"_blank\" rel=\"noopener\">Normelor de aplicare a prevederilor Ordonan\u021bei de Urgen\u021b\u0103 a Guvernului nr. 158\/2005<\/a> privind concediile \u0219i indemniza\u021biile de asigur\u0103ri sociale de s\u0103n\u0103tate, aprobate prin <a href=\"https:\/\/legislatie.just.ro\/Public\/DetaliiDocument\/196682\" target=\"_blank\" rel=\"noopener\">Ordinul ministrului s\u0103n\u0103t\u0103\u021bii \u0219i al pre\u0219edintelui Casei Na\u021bionale de Asigur\u0103ri de S\u0103n\u0103tate nr. 15\/2018\/1.311\/2017<\/a>, este important s\u0103 fie avute \u00een vedere urm\u0103toarele:&nbsp;<\/div>\n<div>&nbsp;<\/div>\n<ul>\n<li>Certificatul de concediu medical pentru \u00eengrijirea pacientului cu afec\u021biuni oncologice \u00een v\u00e2rst\u0103 de peste 18 ani se elibereaz\u0103 persoanei asigurate care, cu acordul pacientului oncologic, \u00eel \u00eenso\u021be\u0219te pe acesta la interven\u021bii chirurgicale \u0219i tratamente, de c\u0103tre medicul specialist al pacientului oncologic.<\/li>\n<li>Medicul specialist din ambulatoriul de specialitate sau spital are dreptul de a acorda concediu medical pentru \u00eengrijirea pacientului cu afec\u021biuni oncologice \u00een v\u00e2rst\u0103 de peste 18 ani cu durat\u0103 de cel mult 45 de zile calendaristice \u00een interval de un an pentru un pacient. Intervalul de referin\u021b\u0103 pentru urm\u0103rirea num\u0103rului de zile de concediu medical este de 12 luni calendaristice de la prima zi de concediu medical.<\/li>\n<li>Pacientul cu afec\u021biuni oncologice \u00ee\u0219i poate exprima acordul numai pentru o singur\u0103 persoan\u0103 la fiecare interven\u021bie chirurgical\u0103, respectiv tratament.&nbsp;<\/li>\n<li>Pentru persoanele asigurate care \u00eendelinesc condi\u021biile prev\u0103zute de lege \u0219i \u00eenso\u021besc pacien\u021bii cu afec\u021biuni oncologice \u00een v\u00e2rst\u0103 de peste 18 ani la tratament\/ interven\u021bie chirurgical\u0103 \u00een \u0163ar\u0103, se elibereaz\u0103 de c\u0103tre casa de asigur\u0103ri de s\u0103n\u0103tate la care este luat\u0103 \u00een eviden\u021b\u0103&nbsp; persoana cu afec\u021biuni oncologice, o adeverin\u021b\u0103 pentru \u00eenso\u021bitor care serve\u0219te la eliberarea concediului medical de c\u0103tre medicul specialist, indiferent de casa de asigur\u0103ri de s\u0103n\u0103tate la care este luat \u00een eviden\u021b\u0103 \u00eenso\u021bitorul.<\/li>\n<li>Adeverin\u021ba se solicit\u0103 utiliz\u00e2nd&nbsp; formularul&nbsp; <a href=\"http:\/\/chrome-extension:\/\/efaidnbmnnnibpcajpcglclefindmkaj\/http:\/\/cas.cnas.ro\/hih\/\/theme\/cnas\/js\/ckeditor\/filemanager\/userfiles\/Anunturi\/2022\/medical\/CERERE_CM_COD_17.pdf\">CERERE<\/a>&nbsp; pentru eliberarea adeverin\u021bei pentru \u00eengrijirea pacientului cu afec\u021biuni oncologice cod 17<\/li>\n<li>\u00cen vederea gestion\u0103rii num\u0103rului de zile de concediu medical pentru \u00eengrijirea pacientului cu afec\u021biuni oncologice, beneficiarul concediului are obliga\u021bia de a transmite de \u00eendat\u0103 o copie a certificatului de concediu medical la casa de asigur\u0103ri care are \u00een eviden\u021b\u0103 pacientul cu afec\u021biuni oncologice<\/li>\n<li>\u00cen situa\u0163ia \u00een care pacientul este \u00eenso\u0163it la tratament pe teritoriul unui stat membru al Uniunii Europene, Spa\u0163iului Economic European \u015fi Confedera\u0163iei Elve\u0163iene sau pe teritoriul altui stat care nu este membru al Uniunii Europene, Spa\u0163iului Economic European \u015fi Confedera\u0163iei Elve\u0163iene, concediul \u015fi indemniza\u0163ia pentru \u00eengrijirea pacientului cu afec\u0163iuni oncologice se acord\u0103 pe baza certificatului de concediu medical eliberat de c\u0103tre casa de asigur\u0103ri de s\u0103n\u0103tate la care angajatorul persoanei asigurate depune declara\u0163ia sau de c\u0103tre cea cu care persoana asigurat\u0103 are \u00eencheiat contractul de asigurare pentru concedii \u015fi indemniza\u0163ii de asigur\u0103ri sociale de s\u0103n\u0103tate, dup\u0103 caz, pe baza actelor doveditoare traduse \u015fi autentificate, \u00een condi\u0163iile \u015fi p\u00e2n\u0103 la durata maxim\u0103 prev\u0103zute de OUG 158\/2005 privind concediile \u015fi indemniza\u0163iile de asigur\u0103ri sociale de s\u0103n\u0103tate, dar nu mai t\u00e2rziu de 15 zile de la data revenirii \u00een \u0163ar\u0103 a pacientului cu afec\u0163iuni oncologice.
